MEDICAL FORMS
DO-NOT-RESUSCIATE ORDERS
Do-Not-Resuscitate (DNR) orders are the most common legal medical forms that patients use should they decide to use a form, but not all states use the same form. Read more to see which forms are used in your state.
POLST FORMS
Physician's Orders for Life-Sustaining Orders (POLST) forms are a new paradigm that is spreading in the United States, but are less commonly used than DNR orders. However, they are often much more clearer in terms of outlining a patient's wishes. Most states have adopted their own system of the POLST form. Read more to see which form your state uses.
LIVING WILLS
Living wills, though similar to the previous medical forms, are sometimes treated differently than DNR or POLST forms and are thus, not often considered as a substitute. Read more to see your state's policies.
