With the large variance that comes with legal medical forms in America, it can be confusing to decide which ones to fill out.

Medical forms that serve a purpose to specify what methods are used to sustain life for a patient are most commonly the do-not-resuscitate (DNR) form and a state's version of the Physician's Orders for Life-Sustaining Treatment (POLST) form.

It should be noted that though these forms are legally accepted by all hospitals, acute care centers, nursing homes, and other healthcare institutions in that respective state, it is a good idea to communicate with your physician to confirm the validity of the form to ensure your wishes will properly be communicated.
