Palliative care focuses on providing relief from the symptoms and stress of a disease. Though typically aimed at those with terminal illnesses, it can be applied and used for patients who are in any stage of their prognosis.

This website provides useful information on the specific resources that will benefit you if you are seeking palliative care based on your location. Choose a hospital, nursing home, or office/clinic, and they can provide you with whether or not they supply palliative care and their contact information.

In the state of Maryland, hospitals that are large enough to support at least 50 patients should have a palliative care program employed and promoted amongst patients. This palliative care team should also provide information about the MOLST form.
