Choose a hospital, nursing home, or office/clinic, and they can provide you with whether or not they supply palliative care and their contact information.
In the state of Maryland, hospitals that are large enough to support at least 50 patients should have a palliative care program employed and promoted amongst patients. This palliative care team should also provide information about the MOLST form.
